The instrument comprises a brief basic assessment and four supplements: Functional (CHA-FS), Mental Health (CHA-MH), Assisted Living (CHA-AL), and Deafblind (CHA-Db). The target population is adults living in the community and a variety of residential care settings. This book contains introductory material; an item-by-item guide to the basic CHA assessment form; triggers and algorithms for the four supplements; item-by-item guides for each of the four supplemental assessment forms; the basic assessment form; and all four supplemental assessment forms.
